- $600 per year — avoid it by completing the tobacco affidavit in Workday during open enrollment, or the BCBS Tobacco Cessation program

- Avoid the surcharge by completing the tobacco affidavit in Workday during open enrollment.
- Or enroll in and complete the BCBS Tobacco Cessation program.
